Instructions
The following forms are fillable using Adobe Acrobat format and do not require any other software for use.
Follow these steps to prepare using these forms in Adobe Reader. If connected to a network, verify with a Network Administrator the Adobe file may be downloaded.
| 1. | Click here to download Adobe Reader. Once downloaded, locate the setup program on your computer, double-click on the file, and follow the instructions to install. This only needs to be done once to access all forms. This program is virus-free.
If Adobe Reader has been previously downloaded, continue with step 2.

| 2. | Once the program is installed, exit and return to this page and select a form category from the list on the right. Select the desired form to open and complete on the computer screen.

Help in Completing Forms
Use the Tab key to move from field to field within a form.
Hover the cursor over a fillable field to view help text associated with a field.
Check Boxes on Forms
Tab to a check box. Check boxes will appear to have a dotted line surrounding it when active. To place a check mark in the box, press the space bar or click on the check box using the mouse. To remove a check mark, press the space bar or click on the check box using the mouse a second time. The check mark will be removed. | These forms are provided for your convenience. The circuit clerk's office in your local county may be able to help you complete Adult Abuse, Child Protection, Small Claims, Family Access, and certain Probate forms. However, your circuit clerk's office will not provide legal advice. You may need to seek legal counsel if you are unable to complete or understand these forms.
