Rule 12: Required Civil Case Information Supplement

In a civil appeal from a circuit court, except a post-conviction appeal, and in all workers' compensation appeals from the Labor and Industrial Relations Commission, the appellant shall complete and file with the Notice of Appeal a Civil Case Information Supplement Form using the form designated for this purpose and made a part of this rule. The appellant shall file the Notice of Appeal and the Civil Case Information Supplement Form with the clerk of the circuit court or the secretary of the Labor and Industrial Relations Commission and serve a copy on each respondent. The circuit clerk or secretary of the Labor and Industrial Relations Commission shall forward the Notice of Appeal, the Civil Case Information Supplement Form and any attachments to this court. All Notices of Appeal shall be accompanied by a copy of the judgment or order appealed from. Failure to file the required Supplement to the Notice of Appeal may result in the dismissal of the appeal or some other sanction.
