Case Information:
Alias Names Associated to Searched Juvenile: Select this link to view alias names for the juvenile as found in the Department of Social Services databases in a new Web page. The first name that displays will be the Juvenile's true name. Additional name(s) listed will be the Juvenile Alias Name(s).
DYS Student Number: A unique number assigned that is an identifier for the DYS student.
Commitment Date: The date the court of jurisdiction ordered commitment. If the juvenile is under an interstate compact, this information will not display but contact information will.
Commitment County: The county from which the court order was issued.
Commitment Offense: A description of the offenses (up to three) from the court order issued.
Placement/Facility Name(s): The name of the last three facilities or residences where the juvenile received services.
Placement Begin Date: The date placement began.
Placement End Date: The date placement ended.
Aftercare Placement: A Y (Yes) or N (No) indicator that the juvenile was placed in aftercare.
Discharge Date: The date the juvenile was discharged from placement.
Discharge Type: A description in how the juvenile was discharged from placement.
Escapee: A Y (Yes) or N (No) indicator if the juvenile has escaped from placement or a facility.
Absconder: A Y (Yes) or N (No) indicator if the juvenile has absconded from placement or a facility.
Flight History:
- Type of Flight: A description of the juvenile’s flight from placement or a facility.
- Date of Flight: The date related to the type of flight.
Case Details:
Does Agency Have Custody: A Y (Yes) or N (No) indicator if the juvenile is in a DYS-controlled location or a location that is not controlled by DYS (such as aftercare).
NOTE: Most youth are committed to the physical care and supervision of DYS. However, some committed youth receive only community supervision and remain in the physical custody of the guardian. Aftercare occurs AFTER residential placement and the youth is receiving services while living with the guardian.
Physical Custodian: The name of the person to whom the Court has awarded physical custody.
Living Arrangement: The name of the person that the court has appointed as Legal Custodian or person with whom the youth is placed. There may be times when the youth will be placed in a location other than with the Legal Custodian; for example, when the victim resides in the home.
Address: Most recent address of Legal Custodian or location where the juvenile resides.
Date: The date of the most recent address of Legal Custodian.
MOSIS Number: The juvenile’s Missouri student identification number as provided by DESE.
Domicile School District: The school district in which the juvenile’s Legal Guardian resides.
Educational Disability: When a juvenile is committed to DYS care, they gather information from past school districts regarding special education services for a handicapping condition. The district must make a diagnosis in order to deliver services. When the district determines that information is sufficient to continue services, they will enter a handicapping condition and date of diagnosis into the DYS system. The “Educational Handicap” and “Diagnosis Date” make up the IEP plan.
Diagnosis Date: The date that the educational handicap was diagnosed.
GED Description: Consists of 5 separate subjects; Math, Reading, Science, Social Studies, and Writing, and a Total. The following are associated to each subject:
- GED Score: The score for the last attempt at GED. Total Score will be the total of the 5 subjects.
- Pass/Fail: A Y (Yes) or N (No) indicator as to whether the juvenile passed or failed the GED.
- Date Scores Posted: The date the GED scores were posted.
Date Graduated High School: The date the juvenile graduated from high school.
Safe School Act Violations: Description of applicable Safe School Act violations, which include only the 9 serious (deadly) offenses precluding a juvenile from returning to a regular school placement. The school has the final option to allow the youth to return if an alternative program is available.

Worker/Contact Name: The Classification Officer or Service Coordinator assigned to the Youth Services case.Worker/Contact Phone Number: The phone number of the worker named.
