Notice of Appeal: This form is filed with the court as an application to a higher (appellate) court to review an order or judgment of a lower trial court when permitted by law. This is filed with the clerk of the court where the case was tried.

Civil Case on Appeal - Legal File Certified Copies: This form is used by a person who is appealing a circuit court judgment to ask the circuit clerk to make official copies of documents.

Execution/Garnishment/Sequestration - Application and Order: This form is used by a person who is filing an application for execution, garnishment and/or sequestration to seek the courts assistance in collecting a money or property judgment due the applicant.

Income and Expense Statement:
This form is used to obtain information from a person regarding their income and expenses in a dissolution (divorce) of marriage, legal separation, modification or paternity case. (Contains no fillable fields.)

Statement of Marital and Non-Marital Assets and Debts: This form is used to obtain information about marital property. Marital property is acquired by either spouse during the marriage. The form provides explanations of each type to be listed or excluded. (Contains no fillable fields.)

Interrogatories to Garnishee: If the court has entered a judgment for money, this form is used to ask the court to issue a garnishment order against the defendant. The interrogatories ask the garnishee to list all property or money owed to or held for the defendant and to state the name of anyone else who owes property or money to the defendant.

Petition for Review of SATOP Assessment Assignment Recommendation: When a person is dissatisfied with the assessment assignment recommendation made by a substance abuse traffic offender program provider, they may file a “Petition for Review of SATOP Assessment Assignment Recommendation.” Along with the petition, the person must include a copy of the “SATOP Offender Assignment” (form AA or new form A3).

Motion to Vacate, Set Aside, or Correct the Judgment or Sentence:
This form is used to ask the court to vacate, set aside or correct a judgment or sentence in a criminal case where the crime was a felony. (Contains no fillable fields.)

Notice of Civil Action to Recover Damages: This form is used by a person who has purchased or leased merchandise for personal, family or household purposes and has suffered a loss of money or property because of an unlawful action or method employed in the selling or leasing of such merchandise under section 407.025, RSMo.

Satisfaction of Judgment:
Once a defendant has paid-off a judgment, this form is used by the person receiving the money or property to let the court know that the judgment has been paid. Do not use this form to satisfy a judgment that contains an ongoing obligation, i.e. child support or maintenance. This form cannot satisfy any child support or maintenance owed to the State of Missouri.

Petition for Establishing Record of Birth: After an application to the Bureau of Vital Records for a certificate of birth has been rejected, this form is used to file an application with the court for an order to be issued directing the Bureau of Vital Records to issue a certificate of birth in accordance with Section 193.115, RSMo. Documentation, as specified within the attached petition and as required by statute, must be filed with the petition.
