In re:
(1) Correction of order of May 19, 2016, repealing subdivision 81.04(a), entitled "Filing the Notice of Appeal," of Rule 81, entitled "Appeals."
(2) Repeal of Form No. 8-A, entitled "Notice of Appeal," and in lieu thereof adoption of a new Form No. 8-A(1), entitled "Notice of Appeal to Supreme Court of Missouri;" Form No. 8-A(2), entitled "Notice of Appeal to Missouri Court of Appeals – Civil;" and Form No. 8-A(3), entitled "Notice of Appeal to Missouri Court of Appeals – Criminal."
ORDER
1. By order of May 19, 2016, this Court adopted a new subdivision 81.04(a) of Rule 81 to be effective January 1, 2017. The Court hereby corrects the order so that, as corrected, effective January 1, 2017, subdivision 81.04(a) shall read as follows:

81.04 APPEALS, WHEN AND HOW TAKEN -- CROSS APPEALS -- DOCKET FEES
(a) Filing the Notice of Appeal. If an appeal is permitted by law from a trial court, a party may appeal from a judgment, decree, or order by filing with the clerk of the trial court a notice of appeal. No such appeal shall be effective unless the notice of appeal shall be filed not later than ten days after the judgment, decree, or order appealed from becomes final.
The notice of appeal shall specify the parties taking the appeal, the judgment, decree, or order appealed from, and the court to which the appeal is taken. For this purpose, the appellant shall use the relevant version of Civil Procedure Form No. 8, as the case may be.
2. It is ordered that effective January 1, 2017, Form No. 8-A be and the same is hereby repealed and a new Form No. 8-A(1), Form No. 8-A(2), and Form No. 8-A(3) adopted in lieu thereof to read as attached hereto.

3. It is ordered that notice of this order be published in the Journal of The Missouri Bar.
4. It is ordered that this order be published in the South Western Reporter.
