ORDER
1. The form attached to this order, having been prepared by the State Judicial Records Committee and reviewed by the Court, is hereby approved for distribution. The form approved supersedes any similar form previously approved by this Court.
2. The office of state courts administrator shall distribute the form attached to this order to each circuit clerk and such other persons as may be appropriate.
