In re:

Repeal of Form No. 8-A(1), entitled "Notice of Appeal to Supreme Court of Missouri;" Form No. 8-A(2), entitled "Notice of Appeal to Missouri Court of Appeals – Civil;" and Form No. 8-A(3), entitled "Notice of Appeal to Missouri Court of Appeals – Criminal," and in lieu thereof adoption of a new Form No. 8-A(1), entitled "Notice of Appeal to Supreme Court of Missouri;" Form No. 8-A(2), entitled "Notice of Appeal to Missouri Court of Appeals – Civil;" and Form No. 8-A(3), entitled "Notice of Appeal to Missouri Court of Appeals – Criminal."


1.  It is ordered that effective July 1, 2019, Form No. 8-A(1), Form No. 8-A(2), and Form No. 8-A(3) be and the same are hereby repealed and a new Form No. 8-A(1), Form No. 8-A(2), and Form No. 8-A(3) adopted in lieu thereof to read as attached hereto.


2.  It is ordered that notice of this order be published in the Journal of The Missouri Bar.

3.  It is ordered that this order be published in the South Western Reporter.
