Motion and Affidavit in Support of Request to Proceed as a Poor Person: This form is used by a party when he/she cannot pay the court costs to make a request to the court to waive court costs and to inform the court of his/her income and expenses under oath. The judge will determine whether the fees will be waived.

Non-Party Request for Transcript: This form allows anyone who is not a party to a case to request a redacted or unredacted transcript(s) for a particular case. If the non-party has good cause for the request, then a judge may order the release of the requested redacted or unredacted transcript(s).
Notice of Invalid Lien: This form is used by a person who has had a consensual common law lien filed against them to notify the court that the lien is invalid.

Request to Establish or Modify Debit Account: This form may be used by an attorney or law firm to request that the court establish a debit account for them. The completed form with payment should be returned to the circuit clerk of the court where the debit account will be established. The debit account MUST have funds prior to using the account. This form can also be used to modify and existing debit account with the court.

Redaction Certification: These forms are filed with the court to certify that all documents in a submission comply with all redaction requirements of Rules 19.10, 55.025, or 84.015.
Motion to Correct Redaction: These forms are filed with the court to inform the court that a document has been insufficiently redacted and to request the clerk raise the security level of a document that contains insufficiently redacted confidential information until the court rules on the motion. Forms include an option for filing as registered judicial officer or immediate family member, as defined in Section 476.1300, RSMo, shielded under the Judicial Privacy Act and allows for multiple documents.
