Repeal of Form No. 5, entitled "Summons for Personal Service Outside the State of Missouri (Except Attachment Action)," and in lieu thereof adoption of a new Form No. 5-A, entitled "Summons for Personal Service Outside the State of Missouri (Except Attachment Action)," a new Form No. 5-B, entitled "Summons for Personal Service Outside the State of Missouri For Motions to Modify Parenting Time (Except Attachment Action)," and a new Form No. 5-C, entitled "Summons for Personal Service Outside the State of Missouri For Motions to Modify Child Support Only (Except Attachment Action)." 

 
ORDER

1. It is ordered that effective January 1, 2016, Form No. 5 be and the same is hereby repealed and a new Form No. 5-A, a new Form No. 5-B, and a new Form No. 5-C adopted in lieu thereof to read as attached.
