Repeal of Form No. 37A, entitled "Uniform Citation," and in lieu thereof adoption of a new Form No. 37A, entitled "Uniform Citation."
ORDER
1. It is ordered that effective July 1, 2015, Form No. 37A be and the same is hereby repealed and a new Form No. 37A adopted in lieu thereof to read as attached.
