In re:

Correction of Form 14 and parenting plan as contained in forms for use by parties not represented by counsel in certain proceedings.


1.  By a December 23, 2008, order this Court approved certain forms prepared by its Committee on Access to Family Courts.   Effective April 1, 2009, line 2(c) of the Form 14 attached to the previous order is corrected to read: "2c. ADJUSTMENT - Support obligation for other children who primarily reside with each parent (Line 2c(2) minus Line 2c(3))".  Similarly, paragraph 8 on page 4 of the Parenting Plan Part B - Support of Children form attached to the previous order is corrected to reference "Section 213 (d)(1)(A)" in lieu of "Section 213."

2.  It is further ordered that notice of this order be published in the Journal of The Missouri Bar.

3.  It is further ordered that this order be published in the South Western Reporter.
