In re:

Repeal of Form No. 8-A, entitled “Notice of Appeal to Supreme Court of Missouri,” Form No. 8-B, entitled “Notice of Appeal to Missouri Court of Appeals,” and Form No. 8-D, entitled “Unemployment Compensation Notice of Appeal to Missouri Court of Appeals,” and in lieu thereof adoption of a new Form No. 8-A, entitled “Notice of Appeal,” and a new Form No. 8-B, entitled “Unemployment Compensation Notice of Appeal.”


1.  It is ordered that effective January 1, 2005, Form No. 8-A, Form No. 8-B, and Form No. 8-D be and the same are hereby repealed and a new Form No. 8-A and a new Form No. 8-B adopted in lieu thereof to read as follows:

2.  It is ordered that notice of this order be published in the Journal of The Missouri Bar.
