In re:

Repeal of Civil Procedure Form No. 8-A, entitled "Notice of Appeal to Supreme Court of Missouri," and Civil Procedure Form No. 8-B, entitled "Notice of Appeal to Missouri Court of Appeals," and in lieu thereof adoption of a new Civil Procedure Form No. 8-A, entitled "Notice of Appeal to Supreme Court of Missouri," and a new Civil Procedure Form No. 8-B, entitled "Notice of Appeal to Missouri Court of Appeals."
ORDER

1. It is hereby ordered that effective July 1, 2000, Civil Procedure Form No. 8-A and Civil Procedure Form No. 8-B be and the same are hereby repealed and a new Civil Procedure Form No. 8-A and a new Civil Procedure Form 8-B adopted in lieu thereof to read as follows:

The clerk shall transmit a copy of the notice of appeal together with the docket fee to the clerk of the Supreme Court. The clerk shall then fill in the memorandum below. (See Rules 81.08 and 30.01(h) and (i)).
