In re:

Forms for Judgment and Decree of Dissolution of Marriage, Paternity, Custody and Support, and Related Instructions


1.  The attached forms for judgment and decree of dissolution of marriage, paternity, custody and support, and related instructions, having been prepared by this Court's Committee on Access to Family Courts and reviewed by the State Judicial Records Committee and this Court, are hereby adopted and approved for distribution and use.  The forms are effective July 1, 2018, and may be used prior thereto.  These forms supersede any similar forms and instructions previously approved by this Court.


2.  It is ordered that effective July 1, 2018, Form CAFC311, entitled "Answer to Father's Petition for Declaration of Paternity, Custody and/or Support" and Form CAFC312, entitled "Answer to Mother's Petition for Declaration of Paternity" be and the same are hereby repealed.

3.  It is ordered that notice of this order be published in the Journal of The Missouri Bar.

4.  It is ordered that this order be published in the South Western Reporter.
