How to use the forms:
- You must save each form before you begin entering information in the form fields. Right click the link for a form and save it to your computer before you begin entering information in the form fields. If you are downloading the form from a public computer, be sure to save the form to a personal device, such as a thumb drive.
- For fillable PDF documents, you must use Acrobat Reader DC (or Adobe Acrobat DC) to ensure the forms work as intended. If you do not have Acrobat Reader DC, a free edition can be downloaded from the Adobe website.
- If you have technical difficulties completing the forms online, you may print all of the blank forms and fill them out by hand to file with the court. Please check with your local court for further instructions.
- You are responsible for the completeness and accuracy of any information submitted on the forms. Please double-check the information you enter!
- Use the Tab key to move from field to field within a form. Hover the cursor over a fillable field to view help text associated with a field. Check boxes can be selected by pressing the space bar or clicking the box. To remove a check mark, press the space bar or click the check box again. Questions requiring only one check box be selected cannot be unselected.
Federal Income Withholding for Support: The Federal Income Withholding for Support form is a multipurpose form used to:
- Initiate new wage withholdings;
- Revise existing withholdings; and
- Terminate existing wage withholdings.

Form 14 Child Support Amount Calculation Worksheet: The Missouri Child Support Guidelines Worksheet (Form 14) estimates the child support obligation that a court may order a parent to pay toward supporting the child(ren) on a monthly basis. Under Missouri law, there is a rebuttable presumption that the amount of child support, which would result from the use of these guidelines is the correct amount of support to be awarded; however, a court may choose to deviate from the guidelines under certain circumstances. Lien Request: This form is used to file a lien against real property (land and buildings on the land) owned by a person who owes child support or maintenance. The lien must be filed with the court in the county where the property is located.

Affidavit for Termination of Child Support: This form can be used by either parent to stop (terminate) child support. The reason child support is to end must be indicated by checking the appropriate boxes.

support to tell the court that he or she agrees the child support obligation
should end.

support to tell the court that he or she opposes the termination of child
support.

Affidavit of Credit for Child Support and/or Maintenance Owed to Custodial Parent (Partial or Complete Satisfaction of Child Support Judgment Pursuant to Section 454.432, RSMo.): This form is used by a parent receiving child support and/or maintenance to credit the automated child support system for direct and in-kind payments received by the custodial parent. This form cannot forgive any child support/maintenance owed to the State of Missouri.
