In re:  Return to Active Status Application Form Pursuant to Rule 6.06(a)


1.  The application form for return to active status, having been prepared by the Office of Chief Disciplinary Counsel and reviewed by this Court, is hereby approved for use.  The application form is effective December 24, 2019.  The application form supersedes any similar form previously approved by this Court.

2.  It is ordered that notice of this order be published in the Journal of The Missouri Bar.

3.  It is ordered that this order be published in the South Western Reporter.
