Motion for Family Access (Visitation) Forms
How to use the forms:
- You must save each form before you begin entering information in the form fields. Right click the link for a form and save it to your computer before you begin entering information in the form fields. If you are downloading the form from a public computer, be sure to save the form to a personal device, such as a thumb drive.
- You must use Acrobat Reader DC (or Adobe Acrobat DC) to ensure the forms work as intended. If you do not have Acrobat Reader DC, a free edition can be downloaded from the Adobe website.
- If you have technical difficulties completing the forms online, you may print blank forms and fill them out by hand to file with the court. Please check with your local court for further instructions.
- You must sign and file the Redaction Certification form (GN320) listed below. Read about redaction and remote public access to public documents.
- You are responsible for the completeness and accuracy of any information submitted on the forms. Please double-check the information you enter!
- If you are a survivor of domestic violence, visit the Safe at Home website for more information before you complete the forms.
