ORDER

Thomas B. Sullivan III, having filed in this Court a motion to voluntarily surrender his license to practice law in this state wherein Petitioner admits that he is guilty of misconduct including, but not limited to, violation of Rules 6.01, 4-1.16(d), 4-1.15, 4-1.1, 4-1.3, 4-1.4, 4-3.2 and 4-8.1 of the Rules of Professional Conduct; and

Petitioner has not tendered his law license to the Clerk of this Court with said petition but has advised that the same cannot be located; and

The Chief Disciplinary Counsel having filed a report and recommendation recommending that Petitioner's petition for voluntary surrender be accepted and that Petitioner be disbarred; and

The Court, thus advised, finds that Petitioner is guilty of professional misconduct and should be disciplined;

Now, therefore, it is ordered that the surrender of the license of Thomas B. Sullivan III be accepted and that said license be canceled.

It is further ordered that the name of Thomas B. Sullivan III be stricken from the roll of attorneys in this state, that he be disbarred, and that his right and license to practice law in this state is hereby terminated.

It is further ordered that the said Thomas B. Sullivan III comply in all respects with Rule 5.27 - Notification of Clients and Counsel.

Costs taxed to Petitioner.
