In re:

Repeal of Form 37.A, entitled “Uniform Complaint and Summons,” and Form 37.B, entitled “Record of Conviction,” and in lieu thereof adoption of a new Form 37.A, entitled “Uniform Citation,” and a new Form 37.B, entitled “Record of Conviction.”


1.  It is ordered that effective January 1, 2003, Form 37.A and Form 37.B be and the same are hereby repealed and a new Form 37.A and a new Form 37.B adopted in lieu thereof to read as follows:

2.  The office of state courts administrator shall distribute the forms attached to this order to each circuit clerk and such other persons as may be appropriate.

3.  It is ordered that notice of this order be published in the Journal of The Missouri Bar.

4.  It is ordered that this order be published in the South Western Reporter.
