Jury Qualification and Summons Forms: Changes Effective September 29, 2020
By order of the Supreme Court of Missouri, forms relating to jury qualification and summons have been changed and supersede any similar forms previously approved by the Supreme Court, effective September 29, 2020.
The Supreme Court approved the following edits made to the Jury forms known as GN90 and GN95:
The Supreme Court approved the following edits made to the Jury forms known as GN90 and GN95:
- Notification methods: email & text, phone carrier with example of carrier and an added question asking how the juror would like to be notified was add to the questionnaire section.
- Sectional heading, “Biographical Information” was added.
- #4 Added follow up question to the felony question.
- #7 Added follow up question to the physical or disability question.
- #8 Created a separate box to add the grand jury question. Including this question on the form will eliminate the need to have two different forms for petit and grand juries. Also added was the question asking "When:".
- #9 Added follow up question for number of years employed by present employer.
- #9 Added follow up question for Self Employed.
- #16 Added additional option for highest grade level completed.
- #19 Added question if the juror was a foreperson.
- Form GN95 – sentence “Any request to be excused must be made below.”, struck.
- Strike: (located at the bottom of the form): “Additional questions may be continued on a following page”.
