The appendix shall include a separate index that lists separately each document in the appendix. Copies of exhibits or excerpts from the record may be included in the appendix only if the exhibits and the excerpted portions of the record are properly filed and made a part of the record on appeal in accordance with either Supreme Court Rule 30 or 81. Original exhibits may not be included in an appendix.
