All motions for rehearing or transfer shall state at the outset thereof the date of the opinion, the author, and the judges concurring or dissenting. If the case was decided by per curiam order and/or unpublished memorandum opinion, the motion shall so state and a copy of the order and memorandum shall so state and a copy of the order and memorandum opinion, if any, shall be attached to the motion.
