If an eligible defendant is released on bail pending appeal pursuant to Rules 30.16 and 33, the stated conditions of the bond shall include:
1. that the defendant report to and appear before an officer of this court with defendant's surety at a designated time within thirty days after the filing of the notice of appeal and defendant appear thereafter at such time and place as required by the Court of Appeals or by the circuit court;
2. that the defendant submit to the orders, judgment, sentence and process of the Court of Appeals; and
3. that the defendant will not leave the geographical jurisdiction of the Court of Appeals without its written permission.
Further, the bond shall contain the defendant's residence and mailing address and the surety's address.
